he would have insisted that she (should) be here on time.

= he would have asked her to be here on time.

= he would have said to her: "Be here on time."

很明显，insisted后宾语从句表示的是“未必是事实”，而不是“与事实相反”。所以要用现在式虚拟语气（即be式虚拟语气），而不是过去式虚拟语气（即were式虚拟语气）。

原句用were是欠妥的。
