(加拿大):The second is definitely wrong, but I've certainly heard the first used. Although 3) may be 'correct' (I've not heard it in this use), the subjunctive is a bit of an endangered species in English.

I happily say "It's time to go" etc, using the to-infinitive. But I don't think I would say "it's high time to go": I think, for me, it would always be a past indicative after "it's high time" - "it's high time I went" or "it's high time I was going".
