情态动词would的用法难点

下面2013新课标I卷的一道题：The door________ open, no matter how hard she pushed.
A.shouldn’t B. couldn't C. wouldn't D. mightn't
答案是C。请问如何解释这个would的用法？为什么不能用couldn’t呢？

用作情态动词的will和would都可表示拒绝或不愿意（用于否定式）。
▲will not / won’t 表示现在的拒绝：
The car won’t start. 汽车发动不起来。
She won’t open the door. 她拒不开门。
▲would not / wouldn’t 表示过去的拒绝：
The window wouldn’t open. 窗子开不开啦。
The wound would not heal. 伤口老不愈合。
The car wouldn’t start this morning. 今天上午这辆汽车发动不起来。
▲题中的wouldn’t与上面的第二条用法吻合。

wouldn't 在此表示“过去拒绝”(refusal)。
1. L. G. Alexander著 Longman English Grammar《朗曼英语语法》(雷航等译外语教学与研究出版社1991年1月第1版)P448：wouldn't 常用于表示“过去拒绝”，可以引申为事物。
The car wouldn't start this morning.
2. Michael Swan著《牛津英语用法指南》(第3版翻译本)(外语教学与研究出版社2010年4月第1版)P979:
wouldn't 可以用来指过去某一特定场合拒绝做某事。
The car wouldn't again start this morning.
3.日] 绵贯阳等著《现代英语语法例解手册》(修订版))(申勇译科学出版社2006年2月第1版)P436:
would 表示“过去很强的意志”。
The rusty screw wouldn't come loose.
couldn't 无此用法。
