in access to decent work 这是介词短语，不叫介宾短语。这个介词短语修饰 labour market inequalities and gaps，不是仅仅修饰gaps。介词后接名词，access当然是名词，你不该对此有疑问的。only 的修饰范围延伸至句末，等于only if..., 只有..., 我们才会发现... 表示唯一条件。
