have a fear 已经是主谓宾结构了，怎么还能再有宾语从句。你这种想法本身就是一个原则性错误。that引导同位语从句，说明fear 是什么。that such a confession...为定语从句，修饰gibe, that为关系代词指代gibe，并在从句中作to elicit的宾语。
