1. and 前的逗号，仅表示停顿。不用也可。
2. that he is against something that he does not understand是个主语从句，跟前面的词组The punishment of hunger 并列。
3. that he does not understand 是定语从句，修饰something。
4. 谓语部分用单数 is，没有are，这在语法上允许。这是把主语当作一个整体看待的缘故。

hungry为网友输入错误，原文是名词hunger。
The punishment of hunger, and that he is against something that he does not understand, is everything.
二个逗号的作用相当于括号，that从句为说话人对主语所做的解释,带有插入语的性质。所以句子的谓语用单数is，而不是复数的are。根据平行原则，一个名词词组是不能与that从句并列作谓语的。
饥饿的折磨（以及由此导致的它与它无法理解的东西对抗）才是最主要的。
