我个人的看法比较倾向于网友和黎老师的理解,即我觉得这里的 it 是指 my knee,it 的这个用法也就是曹老师说的前照应(代词出现在所指代内容前面)。其实,把这个句子改写一下会更好理解:
When I bend my knee, it hurts. 我弯曲膝盖时,它(=膝盖)就疼。(根据照应的基本规律:凡是可以用前照应的场合也可以用后照应来改写,但反之则不一定)
为什么不宜认为 it 是指代 when I bend my knee 呢?因为 when I bend my knee 是一个“时间”,而时间是不可能“疼”(hurt)的。也就是说,hurt(疼)的主语应该是 my knee,而不应该是其他的东西。
另外,我完全同意刘老师所举的例句,而且归纳也很好,很全面。但我觉得与网友的这个句子稍有不同。比如下面一句:
She won’t like it if you arrive late. 她不喜欢你迟到。
严格说来,这里的 it 不是代词,而是虚义用法的 it,因为 like 是及物动词,它后面原则上应有宾语,但这个句子中的 if you arrive late 是状语从句,所以就在 like 后面用了虚义的 it 来填补这个宾语。这个问题与前面一个网友提的以下问题有些类似:

my knee 在通常情况下是不疼的,只有when I bend it 时,my knee 才疼。所以,it 指代整个when 从句的内容。it 不能单独指 my knee。
