我跟曹老师的看法有所不同。我认为，正确答案应为C.， should have 是 should have sent it out 的省略说法。

这样表达，前后并不矛盾，而且意思很贴切。我们把两个并列分句颠倒过来说，也是一样：

I should have sent out my application form last week, but I didn’t.

我在上周本应该寄出我的申请表的，但实际上没有。

总之，不管怎样说，didn't send out 说的是事实，而should have sent out 表示“当时本因该做到，而实际未做到。”

关于might have to do，意思是“说不定做某事；可能得干某事”，表示对现在情况的推测，比may have to do 表示的可能性略小一些。表示对过去情况的推测，不用 might + 原形，而是用 might + 完成式（用 may + 完成式也可）。

You might have to wait a little while.

你可能得等上一小会儿。
